Completing it ahead of time helps us prepare your chart and verify information for a faster check-in. Why do I need to complete patient forms before my visit?

Filling out your forms ahead of time helps our team prepare for your appointment. It gives us important details about your medical history, medications, and allergies so we can provide safe, personalized care and reduce your waiting time when you arrive.

You can download all patient forms directly by clicking on the button above. Most forms can be filled out electronically and printed, or you may print them to complete by hand and bring them to your appointment.

Print and bring your forms with you or email to info@flaoralsurgery.com 

Please bring your completed forms, a valid photo ID, your insurance card (if applicable), and a list of any medications you are currently taking. This ensures a smooth check-in process.

If you have trouble accessing the forms, call our office and we’ll email or mail them to you. You can also arrive 15 minutes early to complete them in person before your appointment.
